Submitting your request to get started

Step 1:

Log into ServiceDesk.

Step 2:

Select Create New Partner Request.

Step 3:

Enter the following information and click Submit.

1. Product:  Forms Builder
2. Partner:  Formisimo/Zuko‍
3. Additional Comments:  Message of Request and Form Sequence Names/URLs to be tracked

Step 4:

Confirm an email acknowledgement was received for the request.
